• If you anticipate needing a prescription for a controlled substance, your first appointment must be in-person

• Note: We are unable to accept out-of-pocket payments from individuals with Medicaid i.e. Healthy Indiana Plan (HIP), Hoosier Healthwise, etc. due to legal restrictions.
